HPS Athletic Packets Due

Warrior Nation,

I hope you are enjoying a relaxing summer!  As we all know, school is just around the corner and fall sports here at HPS will be kicking off in 2 weeks.  I want to remind anyone who has a son or daughter that will be participating in any fall sport that the HPS athletic packet must be completed and in the athletic office before participation is permitted.  Included in the athletic packet is:

Warrior Athletic Department Sport Participation Packet

OHSAA Pre-Participation Physical Form (must be up-to-date)

OHSAA Authorization Form

Emergency Medical Authorization Form

Assumption of Risk Form

Parental Acknowledgment of Policies Form

Eligibility Checklist

Concussion Information Form

*Sudden Cardiac Arrest and Lindsay's Law Form (New mandated information required to be signed off on by student-athlete and parent/guardian)

If the packet with all forms mentioned above is not completed and in the athletic office, the student-athlete will not be permitted to participate in any practice, scrimmages, or contests when the fall season begins on August 1st (July 31st for football) until this is done.

The coach of your specific sport should have packets that have been handed out or available for you at this time.

If you have any questions or need an athletic packet, they can also be picked up in the HPS Administration Office or the HPS Athletic Office.

God Bless and GO WARRIORS!

Joey Day

Director of Athletics

Achieving academic excellence and athletic prowess are important aspects of education for your children, and Harvest Preparatory School offers your children both. But by far the most important part of a student’s education at Harvest Prep is our emphasis on maintaining a Christ-centered atmosphere.

Our Christian emphasis and chapel services alone set Harvest Preparatory School apart from many other schools. All of our curricula are Christ-centered and emphasize a Christian world-view. Harvest Prep students come from 70 different area churches, and we encourage each student to be involved at their home church.
